It wasn’t a premiere but I remember going to a test screening of a horror film I was working on, and a woman in the audience screamed at one point… I knew every sound in our mix so it was a bit startling but the bean counters loved it.
The classic test screening story has to be JAWS:

Literally co-opted by the Nazis it’s a handheld mechanical calculator, and was used for the worst possible purposes. The back-story of how it works and how the inventor conceived of it is fascinating, despite the circumstances:

        Dunno how I missed this!

        I tried a wind organ from a plastic 44-gallon-style drum one time, but it needed too much wind to get a hoot. That cyclone would’ve helped!
